1. Right-click here and "Save Target As..."
2. Download the document to the directory or folder of your choice.
3. Open and print in Microsoft Word.


PDF format (.pdf)
1. Right-click here and "Save Target As..."
2. Download the document to the directory or folder of your choice.
3. Open and print in Adobe Acrobat (available free here...).

JPEG format (795 kb)
Directions for downloading this file:
(This file "looks" too big because it is a high-res image (300dpi) and may need to be scaled before using)
1. Click here.
2. Right-click anywhere inside the new browser window.
3. Choose "Save Image As", "Save Picture As" or "Download Image to Disk".
4. Choose a location to save the image; then click "Save".
5. Import the jpeg as an image onto the page in a word-processing application like Microsoft Word.
6. Print from your word-processing application.
